Sen. Mitt Romney with a New Mexico senator on Thursday presented a bill to keep electronic cigarettes and other vaping systems away from educational and child care facilities across the country, saying youth use has reached epidemic levels.

"In my home state of Utah, the use of Electronic Cigarette Wholesale has nearly doubled in the last five years, with young Utahns most likely to be introduced to vaping while they are in school," Romney, R-Utah, said. "By banning the use of electronic cigarettes in schools, we are taking an important step to protect the health of young people in Utah and across the nation."

Utah has already taken action, with its Indoor Clean Air Act, to ban smoking cigarettes or e-cigarettes within 25 feet of the entrance to any building with public access, including schools and child care centers, among other places that people frequent.

"A lot of states don't have that," said Ryan Bartlett, with the Utah Department of Health's Tobacco Prevention and Control Program. He said Romney is likely taking something that has worked in Utah and trying to get it implemented at the national level.

Bartlett said that no matter where a policy is enacted, a bill like the Smoke-Free Schools Act of 2019 "would be beneficial."

"It would be good for us to see this implemented in other places to keep more people, including youth, healthy," he said.

The U.S. Centers for Disease Control and Prevention report a nearly 48 percent increase in e-cigarette use among middle schoolers from 2017 to 2018, and a nearly 78 percent increase among high-schoolers during the same time.

It is estimated that there are about 3.6 million "current users" of e-cigarettes in America's middle and high schools, according to the CDC, including an increase of 1.5 million in one year.

Electronic cigarettes, known as e-cigarettes or vapourisers, are battery-powered devices that mimic the action of smoking, offering nicotine (in most cases), but without the toxic effect of tobacco smoke. They’ve been in the UK since 2007, and keep growing in popularity.
More people may be using them, but e-cigarettes are not harm-free. A study published in Environmental Science & Technology in July 2016 identified harmful emissions in the vapour, including possible carcinogens and irritants, though at a much lower level than in conventional cigarettes. The BHF would not advise non-smokers to start smoking e-cigarettes. Public Health England (PHE) estimates they are 95 per cent less harmful than regular cigarettes.

Dr Mike Knapton, Associate Medical Director at the BHF, said: “We would not advise non-smokers to take up e-cigarettes, but they can be a useful tool for harm reduction and to stop smoking.”

According to ASH (Action on Smoking and Health), almost half of the UK’s 2.8 million e-cigarette users are former smokers, suggesting they are helping people to stop smoking.

However, a 2014 study in the New England Journal of Medicine found that “electronic cigarettes may function as a ‘gateway drug’ that can prime the brain to be more receptive to harder drugs.”

Martin Dockrell, Tobacco Control Programme Lead at PHE, said: “We know that e-cigarettes are probably not completely safe, but that’s not the issue. The question is, are e-cigarettes safer than the alternative? And, for almost all e-cigarette users the alternative is smoking, and it's really important that they understand how much safer e-cigarettes are, compared to smoking".

The bottom line is to help children grow up safer and healthier and to increase parents' ability to help them do that, says the investigator leading a $1.4 million state-funded alcohol and substance abuse prevention program.

Investigators at the Medical College of Georgia and Georgia Cancer Center are working with Augusta area schools to prevent or reduce use of two major substances abused by children: alcohol as well as Electronic Cigarettes, whose use by teens is reaching epidemic proportions, according to the Food and Drug Administration.

"We are focusing on alcohol and electronic cigarette use, but more broadly we are working to prevent children from taking up any type of substance abuse, and to help parents better understand the importance of behaviors they are modeling at home," says Dr. Martha S. Tingen.

It's a definite battle, with 1 in 5 middle and high school students using some sort of tobacco product and e-cigarettes now at the top of their list.

"The Centers for Disease Control and Prevention tells us the efforts to prevent youth taking up smoking have been erased with the onset of e-cigarettes," says Tingen, Charles W. Linder Chair in Pediatrics at MCG and associate director for cancer prevention, control and population health at the Georgia Cancer Center at Augusta University.Children don't think e-cigarettes are harmful, she says, but the increased vulnerability of the developing adolescent brain to nicotine addiction has groups like the Food and Drug Administration and Office of the U.S. Surgeon General concerned.

So the investigators are taking substance abuse prevention to children and parents simultaneously. Their effort starts by working with professionals at schools like Augusta's Murphey Middle School to identify children and families who might benefit most; surveying the children and families about how these issues affect their lives; then offering simultaneous but separate, tailored classes for children and their parents, says the prevention researcher.

They ask children questions like when was the first time they remember drinking alcohol, whether they thought it was OK for children their age to drink, and what risks they associate with marijuana use. The questionnaires are anonymous and children only answer questions they feel comfortable answering, Tingen notes

Investigators take what they learn from the students to also develop a targeted, positive social norms campaign for their school, which generates slogans like: "91% of Murphey Middle School Students DO NOT VAPE," rather than focusing on the inverse fact that 9% of the children at this inner city middle school do use e-cigarettes.

E-cigarette vaping with nicotine appears to impair mucus clearance

Electronic Cigarettes Wholesale vaping with nicotine appears to hamper mucus clearance from the airways, according to new research published online in the American Thoracic Society's American Journal of Respiratory and Critical Care Medicine.

In "Electronic Cigarette Vapor with Nicotine Causes Airway Mucociliary Dysfunction Preferentially via TRPA1 Receptors," a team of researchers from the University of Kansas, University of Miami and Mt. Sinai Medical Center in Miami Beach reports that exposing human airway cells to e-cigarette vapor containing nicotine in culture resulted in a decreased ability to move mucus or phlegm across the surface. This phenomenon is called "mucociliary dysfunction." The researchers report the same finding in vivo in sheep, whose airways mimic those of humans when exposed to e-cigarette vapor.
Mucociliary dysfunction is a feature of many lung diseases, including asthma, chronic obstructive pulmonary disease (COPD), and cystic fibrosis. Specifically, the study found that vaping with nicotine impairs ciliary beat frequency, dehydrates airway fluid and makes mucus more viscous or sticky. These changes make it more difficult for the bronchi, the main passageways to the lung, to defend themselves from infection and injury.

The researchers note that a recent report found that young e-cigarette users who never smoked were at increased risk to develop chronic bronchitis, a condition characterized by chronic production of phlegm that is also seen in tobacco smokers.

Dr. Salathe said the newly published data not only support the earlier clinical report, but help explain it. A single session of vaping can deliver more nicotine to the airways than smoking one cigarette. Moreover, according to Dr. Salathe, absorption into the bloodstream is lower, possibly exposing the airways to high nicotine concentrations for prolonged periods of time.

The study also found that nicotine produced these negative effects by stimulating the ion channel transient receptor potential ankyrin 1 (TRPA1). Blocking TRPA1 reduced the effects of nicotine on clearance in both the human cells in culture and in the sheep.
